General Consent and Waiver to Transport Child

For the duration of the Sidekick Camps, I authorize Sidekick Coffee & Books, LLC to transport my minor child, _______________________________, driven by an individual authorized by Sidekick Coffee & Books, LLC or provided by public transportation (Cambus/City Bus). I understand that if my child is late to the program or camp, I may need to provide transportation to the location of the applicable activity or miss camp for that day. I recognize participation in this activity, as with any activity involving motor vehicle transportation, causes my child risk or personal injury or permanent loss. I hereby attest and verify I understand the potential risks, and I have full knowledge of the risks involved in this activity, and I assume any expenses incurred in the event of an accident, illness, or other incapacity, regardless of whether I have authorized such expense. I release any claim on Sidekick Coffee & Books, LLC and their agents, officers, employees, and volunteers from any claim that I could bring on my child’s behalf with regard to any damages, demands, or actions whatsoever, in any manner arising out of this transportation. I have read this entire waiver and authorization form, I fully understand its terms and conditions, and I agree to be legally bound by its terms.

General Liability Waiver

This agreement releases Sidekick Coffee & Books, LLC from all liability relating to injuries that may occur during Sidekick Programs and Camps. By signing this agreement, I agree, on behalf of my minor child, to hold Sidekick Coffee & Books, LLC entirely free from any liability, including financial responsibility for injuries incurred, regardless of whether injuries are caused by negligence. I also acknowledge the risks involved in working on projects, being outdoors, etc. These include but are not limited to injury, exposure to allergens, dehydration, and seasonal outdoor exposure (sunburn, poison ivy reaction, etc.). My minor child is participating voluntarily, and all risks have been made clear to us. Additionally, I have communicated any conditions that will increase my child’s likelihood of experiencing injuries while engaging in this activity, to allow Sidekick to attempt to reasonably accommodate the condition. By signing this agreement, I forfeit all right to bring a suit against Sidekick Coffee & Books, LLC, and its owners, managers, staff, insurers, and other such individuals or entities, for any reason. I and my minor child will also make every effort to follow safety precautions.

Sidekick Coffee & Books, LLC routinely promotes programs and activities involving minors through various media. This includes, but is not limited to newsletters, newspapers, brochures, social media, and displays. In doing so, the names and photos of members may be included to help promote or otherwise share information regarding the program or activity.

Illness
